Ferritin is stored iron, you want it high before pregnancy, when iron demand doubles
Thyroid (TSH)
Thyroid issues affect fertility and early pregnancy. Optimal TSH preconception is under 2.5
Glucose & HbA1c
Uncontrolled blood sugar in early pregnancy raises risk of birth defects
Vitamin D
Most people are deficient. Low Vitamin D is linked to preeclampsia and preterm birth
Immunity & infectious disease
Rubella & varicella immunity
If not immune, vaccinate now, you cannot vaccinate during pregnancy
STI panel
Chlamydia, gonorrhea, syphilis, HIV, hepatitis B & C all affect pregnancy outcomes
Lead levels
Lead affects fetal brain development. Especially relevant in older homes.
Nutrition & additional
Folate / B12
Critical for neural tube development in the first weeks, before you know you're pregnant
Lipid panel (fasting)
Useful baseline, especially with a family history of heart disease
Blood type & Rh factor
If Rh negative, you'll need Rhogam injections during pregnancy
Urinalysis
Screens for kidney function, UTIs, and protein, all relevant to pregnancy health

Some medications are not safe in pregnancy. Your provider needs this list before you conceive so you can make a plan together.
