Rapid swellingFace, hands, feet swelling suddenly — not end-of-day puffiness
No fetal movementAfter 24 weeks: fewer than 10 movements in 2 hours after eating
Intense itchingPalms or soles of feet, especially at night — possible cholestasis, requires a bile acid test
Any vaginal bleedingAny amount, any time — always warrants a same-day call
Regular contractionsBefore 37 weeks — report immediately even if they don't hurt
